New Patients

If you're a new patient, you will be emailed a link to fill out a New Patient Intake Form after your appointment is scheduled. If you are unable to complete it before arriving to your appointment, please arrive 15 minutes early to do so. This must be completed before your treatment begins. Payment is due at the time of treatment. If applicable, please bring records of any imaging and medications/supplements you take; and please make sure you've eaten at least 30 minutes before receiving your treatment. 

Cancellation Policy

Dahms Acupuncture requires 24-hour cancellation notice for all scheduled appointments. Please expect a charge for 50% of the treatment when cancelling with less than 24 hours notice. This policy is in place out of respect for the practitioner and patients. Cancellations with less than 24 hours notice are difficult to fill. By giving last minute notice or no notice at all, you prevent another patient from being able to schedule into that time slot. Please keep in mind that late arrivals may result in a shorter treatment.

Copays and Coinsurance fees vary depending upon your individual plan. Copays are due at the time of treatment.

Please bring your insurance card at the time of your first appointment to ensure a speedy verification process.

*Some insurance plans do not cover adjunctive therapies, such as cupping, and moxibustion. Fees for such therapies are due at the time of service in addition to your copay.
